Complete and Print: Ensure you complete all sections of the form, sign and date it, and print it on single-sided paper.

2. Gather Required Documents:

Proof of U.S. Citizenship: This could be a birth certificate, a previously issued U.S. passport, or a Certificate of Citizenship.

Proof of Identity: This could be a valid driver's license, military ID, or other government-issued photo ID.

Photocopies: Make photocopies of your proof of citizenship and your photo ID.

Passport Photo: You'll need a recent passport photo that meets specific requirements.

3. Pay the Fees:

The fees vary depending on whether you are applying for a passport book, passport card, or both.

Fees are payable to the Department of State.

You'll also need to pay an execution fee to the acceptance facility.
