Please print and read the RELEASE OF LIABILITY, ASSUMPTION OF RISK, and INDEMNIFICATION AGREEMENT above and bring a signed copy the day of the wedding. 
Every guest attending, that’s 18 and older, is required to fill one out. There will be a few extras at the gate incase you have forgotten yours.
