**PLEASE REVIEW AND BOOK THROUGH THIS FORM BY JUNE 5, 2024.**

Our travel consultant, Teresa Davis, will be helping guests make travel arrangements.

Once you have completed the Booking Form, Teresa will put your requested room on hold and send you an invoice that will require your approval. As soon as you have approved the invoice, she will place a deposit on your trip and get your trip booked.

There is a deposit of $200 per room due at the time of booking to hold your reservation. If you wish to purchase flights and insurance, that is also due at the time of booking.

JUNE 5, 2024, is the last date you will be able to book accommodations within our discounted room block. We have reserved a block of rooms for Wednesday, October 23 to Monday, October 28, 2024, and encourage you to book a minimum of three nights to enjoy all that the resort has to offer!

EACH traveler in your party must have a passport and E-tickets for this trip. The name on your passport and E-tickets must match your airline tickets exactly. Passports must be valid for 6 months after travel. Each traveler needs an E-ticket for arrival into Punta Cana AND an e-ticket for departure out of Punta Cana. The e-ticket form should be completed up to 72 hours prior to travel.

The E-Ticket service is completely free of charge. If you are being charged for e-tickets, you are on a fake website.

The closest airport to the Hard Rock Punta Cana is: PUJ - Punta Cana International Airport. You will have the option to select your preferred ground transportation (ride to and from the airport) method when you complete the Booking Form.

When using shuttle service for your transportation to and from the airport, please refrain from other taxi and shuttle services trying to sell their services. Please find the sign for your reserved shuttle service.
